PADI Open Water Diver certificate
A student diver must present the PADI Universal Referral Form for Open Water Diver Certification to a dive instructor. It states that the diver was trained by another organization. The form directs the diving instructor to complete the student's open water dives, sign a statement, and send the form to the student's original instructor, who should certify the diver.
The Universal referral form can be used to get certification. But, PADI demands that you complete the course within 12 month of your last training. This is usually a two- or three day course. You must be able and willing to dive with your buddy up to 60 feet/18 metres.

Statement for medical purposes
To get certified as a diver, you need to complete the Medical Statement to Universal Referral Form. This is an important document to ensure that you are healthy before diving. It is intended to ensure safety underwater. Please answer all questions honestly.
First, get your physician to sign the form. This is required when you start your training as a diver. If you have a new condition, a medical release will be required. A photocopy of your certification card is recommended.
Logbook
A medical statement is required before a diver can open-water dive. This usually happens before certification classes begin. The medical release must be signed by a physician. Changes in medical conditions will require a medical release. PADI can provide a medical release. The PADI Release of Liability/Assumption Of Risk/Non agency Acknowledgement For - General Training is the equivalent of the medical release.
The universal referral form differs from the one in a logbook. It will list the open water dives and knowledge-development sections the student has completed. It also contains a signed statement from the instructor confirming that the diver has taken the required dives. The student is then required to take the form back to their instructor to be certified as a diver.

Completion requirements for courses
The Universal Referral Program is used by most well-known scuba certification organizations. This means that a diver could take the theory, pool and open water courses with the local dive center while completing the open water inspection dives with another agency. This is possible with the help of a diving center that participates in a universal program such as SSI and NAUI or NASDS.
PADI Open Water Diver training requires students to pass the Knowledge Development, and Open Water Dives sections. Additionally, the student diver must provide a medical release signed by his or her physician before diving. A photo must be submitted by the student diver to accompany his/her certification card. Digital images will be accepted.
